BEIJING — China’s big-money push to build ports, rail lines and telecommunications networks around the world — and increase Beijing’s political sway in the process — seemed to be running out of gas just a year ago.Now the program, called the Belt and Road Initiative, has come roaring back. Western officials and companies, for their part, are renewing their warnings that China’s gains in business and political clout could come at their expense.Chinese companies signed Belt and Road contracts worth nearly $128 billion in the first 11 months of last year, according to China’s Commerce Ministry, a 41 percent increase over the same period in 2018. The contracts are mostly for construction and equipment by big Chinese companies using Chinese skilled labor and loans from Chinese banks, although the projects often create jobs for local laborers as well.The latest contracts include a subway system for Belgrade, Serbia; an elevated rail line in Bogotá, Colombia; and a telecommunications data center near Nairobi, Kenya.The return of Belt and Road is likely to raise tensions with the United States, which worries that China is building a globe-spanning bloc of nations that will mostly buy Chinese goods and tilt toward China’s authoritarian political model. The initiative figures into many of the disputes between the two countries over national security and technology.The rush of new Belt and Road contracts follows a public pullback by Chinese officials in 2018 after projects in Malaysia, Sri Lanka, Pakistan and elsewhere were criticized by local officials and others as bloated and costly. China argues that since then, it has fine-tuned practices to trim waste. “We will continue to follow a high-standard, people-centered and sustainable approach to promote high-quality Belt and Road cooperation with partner countries,” Xi Jinping, China’s top leader, said during a visit to Brazil in November.Chinese officials have long presented Belt and Road as a chance to give emerging markets the same kind of world-class infrastructure that has helped make China a global economic powerhouse. Under Belt and Road, state-owned Chinese banks typically lend practically all of the money for a construction project to be carried out by Chinese companies. The borrowing countries are then required to repay the money, often with oil or other natural resources. Officials in the United States and Western Europe have long criticized Belt and Road as predatory, and in recent years, some officials in developing countries began to agree. In 2018, Sri Lanka gave its major port to China after it could not repay loans, while Malaysia halted its own costly Belt and Road projects. Chinese leaders began to acknowledge the criticism. Vice Premier Liu He of China publicly raised concerns in early 2018 about heavy lending by Chinese banks, not just for the Belt and Road Initiative. In the months that followed, Chinese financial regulators clamped down hard on domestic and overseas lending alike. New Belt and Road contracts plummeted, Chinese data showed. China’s financial regulators told the country’s banks to look twice at further lending to poor countries. Top leaders practically stopped mentioning the program.But the credit crunch produced a much broader slowing in the Chinese economy in 2018 than expected. Financial regulators reversed course. That has produced a revival of lending for domestic infrastructure projects and for Belt and Road projects alike. Contracts started to be signed in earnest again in the final weeks of 2018, and momentum built through last year.In recent days, two groups representing Western governments, companies and banks have raised questions about the resurgence of the Belt and Road Initiative. A report released on Thursday morning by the European Chamber of Commerce in China concluded that Chinese-built telecommunications networks and ports are set up in ways that make it hard for European shipping companies, computer software providers and other businesses to compete. A survey by the chamber of its members also found that they had been almost completely excluded from bidding on Belt and Road Initiative contracts, which went mostly to Chinese state-owned enterprises.“It was rather sobering to see that for businesses, it is quite insignificant what we get out of this,” said Joerg Wuttke, the chamber’s president.The Institute of International Finance, a research group in Washington backed mainly by big Western banks, issued a different warning on Monday as part of a broader report on global debt.The institute’s report said that many poor countries in the Belt and Road Initiative now find themselves with sharply increased debt burdens. Many of these countries could barely qualify to borrow money even before they took on the new debt, the report said.The institute’s report also said that 85 percent of Belt and Road projects involved high emissions of greenhouse gases linked to climate change. These projects have included at least 63 coal-fired power plants.The new reports come after a warning issued last year by European International Contractors, a trade group of construction and engineering companies. The trade group cautioned that loans for Belt and Road Initiative projects tend to carry considerably higher interest rates than those from lending institutions like the World Bank. The construction industry group, and also the European chamber, said that the costs of Belt and Road Initiative projects are often greatly underestimated so that they can pass muster with Beijing officials. Poor countries then end up paying for cost overruns, they said.European business groups, which include telecommunications equipment makers, have focused lately on Belt and Road’s emphasis on telecommunications. Many developing countries now have national telecom networks built by two Chinese companies, Huawei and ZTE, that have been big participants in the Belt and Road Initiative. Huawei won a contract last spring to build a large telecom data center in Kenya. The European chamber report said the networks were designed in ways that made it hard for European companies to sell any further hardware or software in these markets. European markets for telecom equipment, by contrast, are often more open, it argued. Huawei, for example, has sought to provide equipment for Germany and Britain. Alongside telecommunications, the biggest security concern in the West about the Belt and Road Initiative has involved China’s construction or expansion of extensive ports. These ports now ring the Indian Ocean and extend up the west coast of Africa and into the Mediterranean.The European Chamber report said that European shipping companies, which have ranked among the world’s largest since the Middle Ages, increasingly find themselves at a competitive disadvantage. The new ports are designed and managed by Chinese state-owned enterprises that are under the same Chinese government agency as Chinese shipbuilders and Chinese shipping companies. Two Female Engineers Are Helping Newborn Babies Survive and Thrive

Sona Shah and Teresa Cauvel are cofounders of Neopenda, a global health tech startup striving to engineer healthcare solutions that give newborns in low-resource settings the healthy lives they deserve. 
Shah and Cauvel met at a Masters Biodesign course at Columbia University. It was there that they started thinking about newborn mortality and why it is so much higher in low-resource settings than it is in the US. After much research and interviews, they came up with Neopenda, a revolutionary, low-cost, low-power wearable sensor that continuously monitors newborns' key vital signs.
In April 2016, the SMPLCT Lab team partnered with The Fu Foundation School of Engineering and Applied Science at Columbia University to judge at 2016 Columbia University Women Entrepreneurs Pitch Competition (WE Pitch). Here is where Sona Shah, Co-Founder of Neopenda, presented Neopenda and won 1st Place at WE Pitch. Cynthia Hellen, Founder of SMPLCT lab recently had a chance to catch up with Sona, and talk startup-life, being a female engineer and the future of Neopenda.
Cynthia Hellen: Where did the idea of Neopenda come from?
Sona Shah: The idea of Neopenda started while my confounder and I were taking a Masters Biodesign course at Columbia University. The course was designed to identify a problem and need, interview key stakeholders, brainstorm solutions, and create a prototype to help solve the problem. We started thinking about newborn mortality and why it is so much higher in low-resource settings than it is in the US. After interviewing many stakeholders and conducting a lot of research, we found that there are too many critically ill newborns and not enough healthcare workers to care for them. We then came up with the idea of a low-cost wearable device that can monitor the vital signs of newborns, and alert when a newborn is in need of immediate attention, so that resources-including nurses’ time-can be used more efficiently and effectively.
Cynthia Hellen: You were a Columbia University student and building a business at the same time. How did you manage both academic and startup life?
Sona Shah: It was tough, but Columbia was a very supportive environment for building a business. While our efforts were mainly focused on Neopenda during our final semesters at Columbia, we valued our academic life at Columbia. We were able to directly apply skills and knowledge we gained in courses to our device and business. Columbia also offered many resources that enabled us to continue our business outside of the Columbia realm- including funding from the Columbia Venture Competition and mentorship.

Cynthia Hellen: After graduating Columbia, what gave you the push to continue with Neopenda?
Sona Shah: My co-founder, Teresa, and I knew that we wanted to continue Neopenda after returning from our first trip to Uganda in September 2015. We had already completed the course where Neopenda began, and continued to work on Neopenda throughout the summer on top of our internships and research jobs. We thought that what we were really missing was the true understanding of whether our device would have a significant impact in a low-resource setting, such as Uganda. We used the funding we received from the Columbia Venture Competition to travel to Uganda and visit many different hospitals. The validation from stakeholders was overwhelming and inspiring- we came back confident that our solution could greatly improve newborn health outcomes. Since then, Teresa and I have been working on Neopenda full time and are committed to getting our solution to the hospitals that need it.  
Cynthia Hellen: Why is Neopenda so important to you?
Sona Shah: I spent 3 months in a rural community in Western Kenya immediately after completing my undergraduate studies, where I was introduced to a beautiful culture that was unfortunately not as privileged as I was growing up. Since then, I’ve been passionate about providing healthcare solutions to these communities, where too many newborns are suffering and dying from preventable causes. While I fully understand the daunting challenges in creating technologies for low-resource settings, I didn’t think there were enough people focusing their efforts on this neglected part of the world. I’m committed to changing that, through Neopenda.  
Cynthia Hellen: What has Neopenda accomplished up to this date?
Sona Shah: Since the idea of Neopenda began in January 2015, we’ve been able to create our first generation device that will be used in our upcoming pilot studies in Uganda. We’ve raised just over $500,000 in funding to continue our work. We’ve participated in an accelerator program called Relevant Health, that helped us develop our MVP and business plan. We’ve established and strengthened several partnerships, including with the Uganda Pediatrics Association and St. Francis Nsambya, a hospital in Uganda where we will perform our first pilot studies. We’ve earned some key validation and recognition, including features from Global Citizen, the Washington Post, and Forbes. While we have a long road ahead of us, we are excited about the progress we’ve made in just a short amount of time.
Cynthia Hellen: What has been the biggest challenges of building a social enterprise?
Sona Shah: Like all startups, we’re challenged to secure the funding to see our company through the arduous early years, especially with the high costs of product development and manufacturing for a hardware/software product. There’s a lot of risk-and forgone salary-when you take the leap to build a startup and the lifestyle can be challenging at times; though completely worth it. It also can be hard, with a social enterprise like ours where our target market is across the world, to stay laser focused on our mission and users. It’s easy to get distracted by all the things that are going on, and lose sight of the reality in Ugandan hospitals for a bit. We try very hard to embed our values and mission in everything that we do as a company, and stay in constant communication with our partners and friends on the ground in Uganda.
Cynthia Hellen: You serve infants that are thousands of miles away. What about serving infants here in the states?
Sona Shah: Our mission is to bring impactful solutions to vulnerable, underserved populations. In Uganda, the neonatal mortality rate is 4.75 times the rate in the United States, and approximately 80% of newborn deaths are from preventable causes. In the United States, deaths from preventable causes have been very nearly eliminated. Hospitals in wealthy nations are well equipped to monitor and treat critically ill newborns, and while we are interested in pursuing some consumer and/or niche clinical applications of our product in the states, our primary focus is to serve low-resource settings where hospitals don’t have anything like what we are building and it will have a huge impact.
Cynthia Hellen: What would be something you have learned being a woman in technology?
Sona Shah: We’ve learned that in tech and entrepreneurship settings we will more often than not be the minority, and have to be fearless and persistent. We’ve also learned that women’s support of each other is invaluable and empowering; from our peers, mentors, and organizations working tirelessly to balance the scales.

Cynthia Hellen: What drives you to wake up and do what you do?
Sona Shah: I love being able to spend my time and energy doing something meaningful and mission-driven. It is very fulfilling to be able to see and shape every aspect of our product and business, and see it through from an idea to a product with palpable impact. Of course, there is always so much to learn, which is part of the fun - we get to dig our hands into so many different areas that I didn’t know much about before. I’m proud of how far we have come in the past year, and grateful for the opportunity to be helping solve important problems and saving lives.
Cynthia Hellen: How do you balance your time with your family and business?
Sona Shah: It’s certainly not easy to start a company, but it’s a lot easier when you have a supportive family. I couldn’t be more thankful for my family and friends and the support they’ve given. It’s very easy to get caught up in the startup life and make your company the only thing you focus on. But I’ve learned that it’s not healthy- for myself or for Neopenda- to do that. Everyone needs time to rejuvenate, and friends and family are my sources for that. You have to find the right balance, and everyone’s balance is different. Neopenda and my family are equally important to me, and both are very intertwined.  
Cynthia Hellen: Where do you see Neopenda in 5-10 years?
Sona Shah: Our vision is for Neopenda to thrive as a global health technology company focused on newborn health. In 5-10 years, we hope to have proven and commercialized our vital signs monitor product in Uganda and many other low and middle income countries, and to be developing other innovations from our pipeline.
Cynthia Hellen: What is your message to those who are looking to start a social enterprise or simply use their skills to create change in the world?
Sona Shah: You can do it. You’ll never work harder in your life, but you’ll also never be happier. It is such a rewarding experience full of opportunities, challenges, sleepless nights, and smiles that can’t be forgotten.

Below is a partial transcript of the Q & A from the "Scrum at Scale: The Path to Agility" presentation that I delivered to the International Institute for Learning, Inc. The questions and brief answers selected for this post provide answers to common questions about whether or not Scrum can work across various industries and organization types.
What we know is that Scrum is being implemented across all domains. Scrum works wherever Scrum is well practiced and well supported. If you are interested in increasing business value and business agility, decreasing waste and time-to-market, or future-proofing your organization, you should be interested in Scrum.     Q: Hello. Thank you for the opportunity. I work for the public sector in Cape Verde, a small African country on the west coast. Considering the projects complexity in the public sector, involving many stakeholders, changing scope and short cycles. What would be your thoughts on how agile practices can help improve public sector project management. Thank you.
Jeff: In Kenya, a World Bank funded organization gets three times the work in 1/3 the time with Scrum. As a result, the World Bank increased their budget for this organization from $5M to $15M this year.     Q: Can Scrum work successfully in Construction Management? If so how?
Jeff: Yes, there are a number of examples. A large bridge in the Netherlands was built with Scrum. Tom Auld from the Collaborative Leadership Team uses Scrum to flip houses. You can also Google Scrum in construction for more details and examples.     Q: Jeff's book and his talk has great examples but Scrum seems more appropriate to process organizations with a pile of work to do. Where is the big picture to see how the items in the pile come together to create a large service or object?
Jeff: Systematic, Lockheed, Raytheon and others do massive billion dollar projects for half price with Scrum. You need training, a disciplined product owner team and strong leadership to coordinate and turn a huge vision into actionable backlog items.     Q: Can you use scrum for enhancements or bug fix in IT? When it does not fit producing a product following full SDLC process.
Jeff: Scrum is widely used in support teams, everywhere from call centers to help desks. Google this and get Scrum training.     Q: How do you avoid integration issues when deliverables are coming faster from different teams? Who is responsible for governance and any incompatibility/integration issues between different deliverables that, in some cases, are meant to work in harmony for a bigger objective?
Jeff: Amazon delivers a new feature to production every 10.6 seconds with 1000 Scrum teams. That's why they have all the money. You need to get Scrum training.     Q: I totally agree with your emphasis on Product Development; However, as a product already in production, is waterfall not the better way to go as profits become important. To simplify, change = change in cost.
Jeff: As Systematic has shown at CMMI Level 5, all projects cost half as much with 40% fewer defects plus or minus 3%. If you want less cost use Scrum.     Q: You provided examples how SCRUM helped organization doing repetitive work (e.g. toyota will keep doing cars). How can SCRUM help organizations who have different deliverables (e.g. IT services company that will deliver different requirements to different clients in different domains) and how can you avoid integration issues if you have scrums of scrums in this case?
Jeff: In our Scrum@Scale class we show how to do this. A deliverable is a set of backlog prioritized by the Product Owner. Teams pull that backlog and deliver a fully integrated increment of that backlog every sprint or more often.     Q: With so many teams, how do you manage load balancing between the teams and avoid under-utilization?
Jeff: Load balancing is managed by the teams and is largely automatic in Scrum. That is the power of self-organization. Load balancing is a waterfall problem, not a Scrum problem.     Q: Any key differentiators between SAFe vs SoS?
Jeff: SAFe is an IT framework that is useless in sales, marketing, and other areas. It is heavy weight with a lot of waste and will never get you twice the work in half the time. SAFe also does not address organizational incremental change which is essential for business agility.     Q: Can you apply scrum to hardware as well as software?
Jeff: Most of our clients today are hardware companies so the answer is yes. Check out Scrum Inc. ScrumLab for many online courses on Scrum in Hardware.     Q: Are you aware of scrum being implemented in higher education?
Jeff: Scrum in widely implemented in higher education. You can Google to find examples.     Q: Which kinds of organizations are not suitable for Scrum?
Jeff: Organizations that want to continue to do half the work in twice the time will not be happy with Scrum. In every organization about 30% of the people do not want to change.     Q: Do you believe all projects can be and should be done using Scrum, or are there some that should still follow Waterfall?
Jeff: Any project that you want to deliver twice the work in half the time with higher quality should be Scrum. The leading experts on process control on the planet have pointed out that waterfall is a predictive process control system suitable only for projects with less that 4% requirements change during development. Otherwise the projects blow up and are late.     Q: Do you think SCRUM could ever work in a University environment?
Jeff: Yes, Scrum can work in  a university environment if people are willing to change (and some are not). Google Scrum in Research.     Q:Where do you see (which industries) Waterfall working better than Agile methodology?
Jeff: Software companies will have difficulty surviving without Scrum today. Hardware companies are finding the same thing. Other domains will learn as they get disrupted by newer technologies. If they don't change they will be replaced by robots.     Q: I work at GlobalFoundries as a product development engineer and a team leader. It seems to me as our whole organization is using SCRUM without even realizing it - How would you suggest implementing official SCRUM methods without disrupting our current quasi SCRUM, if you will?
Jeff: Implement Scrum@Scale where you start with a small subset of teams. When they can do twice the work in half the time, roll that model out across the organization.     Q: How to start the implementation of SCRUM in a big and complex oil company with a very rooted culture (tradition). Where and how to start?
Jeff: Scrum@Scale starts with management education and setting up an Executive Action Team which will lead the transformation.     Q: How would you approach fixing the issues with Health Care in the United States using SCRUM methodologies?
Jeff: Healthcare needs total disruption and Scrum is the tool to do it with. I spent 23 years in healthcare implementing Scrum. In fact, I developed Scrum while working on healthcare research projects that were heavily funded by the NCI.     Q: I am very involved as the President of a not for profit home owners association ran by a team of 5 volunteer BODs. Are you aware of any Agile implementations in similar organizations?
Jeff: There is a paper on Scrum in Church which shows how to run a non-profit organization with Scrum. Large banks and our venture capital group implement this model.     Q: Can SCUM be applied to R&D in the Health Care Sector?
Jeff: After spending 11 years in research as a medical school professor and 12 years as CTO of two leading healthcare software companies, I can definitely say yes. Google Scrum in research.

Google this and get Scrum training.     Q: How do you avoid integration issues when deliverables are coming faster from different teams? Who is responsible for governance and any incompatibility/integration issues between different deliverables that, in some cases, are meant to work in harmony for a bigger objective?
Jeff: Amazon delivers a new feature to production every 10.6 seconds with 1000 Scrum teams. That's why they have all the money. You need to get Scrum training.     Q: I totally agree with your emphasis on Product Development; However, as a product already in production, is waterfall not the better way to go as profits become important. To simplify, change = change in cost.
Jeff: As Systematic has shown at CMMI Level 5, all projects cost half as much with 40% fewer defects plus or minus 3%. If you want less cost use Scrum.     Q: You provided examples how SCRUM helped organization doing repetitive work (e.g. toyota will keep doing cars). How can SCRUM help organizations who have different deliverables (e.g. IT services company that will deliver different requirements to different clients in different domains) and how can you avoid integration issues if you have scrums of scrums in this case?
Jeff: In our Scrum@Scale class we show how to do this. A deliverable is a set of backlog prioritized by the Product Owner. Teams pull that backlog and deliver a fully integrated increment of that backlog every sprint or more often.     Q: With so many teams, how do you manage load balancing between the teams and avoid under-utilization?
Jeff: Load balancing is managed by the teams and is largely automatic in Scrum. That is the power of self-organization. Load balancing is a waterfall problem, not a Scrum problem.     Q: Any key differentiators between SAFe vs SoS?
Jeff: SAFe is an IT framework that is useless in sales, marketing, and other areas. It is heavy weight with a lot of waste and will never get you twice the work in half the time. SAFe also does not address organizational incremental change which is essential for business agility.     Q: Can you apply scrum to hardware as well as software?
Jeff: Most of our clients today are hardware companies so the answer is yes. Check out Scrum Inc. ScrumLab for many online courses on Scrum in Hardware.     Q: Are you aware of scrum being implemented in higher education?
Jeff: Scrum in widely implemented in higher education. You can Google to find examples.     Q: Which kinds of organizations are not suitable for Scrum?
Jeff: Organizations that want to continue to do half the work in twice the time will not be happy with Scrum. In every organization about 30% of the people do not want to change.     Q: Do you believe all projects can be and should be done using Scrum, or are there some that should still follow Waterfall?
Jeff: Any project that you want to deliver twice the work in half the time with higher quality should be Scrum. The leading experts on process control on the planet have pointed out that waterfall is a predictive process control system suitable only for projects with less that 4% requirements change during development. Otherwise the projects blow up and are late.     Q: Do you think SCRUM could ever work in a University environment?
Jeff: Yes, Scrum can work in  a university environment if people are willing to change (and some are not). Google Scrum in Research.     Q:Where do you see (which industries) Waterfall working better than Agile methodology?
Jeff: Software companies will have difficulty surviving without Scrum today. Hardware companies are finding the same thing. Other domains will learn as they get disrupted by newer technologies. If they don't change they will be replaced by robots.     Q: I work at GlobalFoundries as a product development engineer and a team leader. It seems to me as our whole organization is using SCRUM without even realizing it - How would you suggest implementing official SCRUM methods without disrupting our current quasi SCRUM, if you will?
Jeff: Implement Scrum@Scale where you start with a small subset of teams. When they can do twice the work in half the time, roll that model out across the organization.     Q: How to start the implementation of SCRUM in a big and complex oil company with a very rooted culture (tradition). Where and how to start?
Jeff: Scrum@Scale starts with management education and setting up an Executive Action Team which will lead the transformation.     Q: How would you approach fixing the issues with Health Care in the United States using SCRUM methodologies?
Jeff: Healthcare needs total disruption and Scrum is the tool to do it with. I spent 23 years in healthcare implementing Scrum. In fact, I developed Scrum while working on healthcare research projects that were heavily funded by the NCI.     Q: I am very involved as the President of a not for profit home owners association ran by a team of 5 volunteer BODs. Are you aware of any Agile implementations in similar organizations?
Jeff: There is a paper on Scrum in Church which shows how to run a non-profit organization with Scrum. Large banks and our venture capital group implement this model.     Q: Can SCUM be applied to R&D in the Health Care Sector?
Jeff: After spending 11 years in research as a medical school professor and 12 years as CTO of two leading healthcare software companies, I can definitely say yes. Google Scrum in research.
